H&P Pro Notebook (Extra Space for Note-taking) – Medical History and Physical Notebook for medical students, residents, nurse practitioner NP, and physician assistants PA

This notebook was great to have for clinical rotations! Made it easier to chart and present. Love the extra space to write notes in this version. Would recommend to all students!

I like the perforated pages so that it is easy to ensure patient privacy. My only dislike is that by keeping the format when the note book is fully open moving from left to right, patient information is spread over two pages so that if you tear out anything, one patient Meds section and following is on the 1st page of another patients. While I may not order again because of paper usage the notebook is overall a 5/5 because it is able to fit into a white coat or event a FIGS jacket pocket.

I bought this when I was just starting as an NP at an Urgent care. This would guide me what to ask and I get to write things down so I won’t forget.

I should’ve read the reviews! Not only are the pages upside down, I’m pretty certain the anion gap calculation formula on the badge cards is incorrect! Beware.

Overall, it's pretty standard in layout for basic reports. I don't find the Social History/Family History as important so I usually retitle those boxes to "Lines/Drains" and "Skin".Could also use a space for Code Status somewhere at the top.I wish the paper was thicker as using any highlighter/marker/inky pen will BLEED to the page behind which leaves me skipping a page for a new report...I (personally) wish the notebook was BIGGER like at least the size of a composition notebook since I like to include extreme details on the patient's overall hospital stay. I have to write in tiny font with the notebooks current size.

Helps me remember to ask all of the history. This grey cover gives the most writing space for the HPI.

I got this for my NP clinicals, and it's exactly what I was looking for. It's the perfect size and has a great structure for my notes that will help me present to my preceptors.

H&P helps me start and finish my day organized. Working in the medical field these days we all know how chaotic our day can be, but this product has absolutely changed the flow of my days! For better day to day use I use page number tags and put the patients room number on them for easier access!
